FDA License to Operate (LTO) + Medical Device Registration (CMDR) - Electromedical/Irradiation Equipment
Manufacturing X-ray, CT/PET scanner, MRI, ultrasound or other electromedical/irradiation equipment requires an FDA LTO as a medical device establishment plus a CMDR, and separate radiation-source clearance from the DOH/PNRI where applicable.
